Preliminary Research Agreement

   I, _____________________________________, herewith authorize Croatian Roots of Salisbury, CT and its representative in Croatia, Sanja Frigan-Ciuha, to conduct preliminary genealogical research for my family name: _________________________. I further authorize Croatian Roots of Salisbury, CT and its representative in Croatia, Sanja Frigan-Ciuha, to request, access and research data on my family in the archives of the Republic of Croatia, City of Zagreb and other local municipalities' offices in Croatia, as needed for the purposes of this research.

   The preliminary genealogical research means that the purpose of the research is to establish the existence and location of the family records of my ancestors in Croatia. I agree to pay for that service a non-refundable fee of US $90.00 (ninety dollars) per surname searched.

   I hereby attach all information known to me about my ancestors, and understand that the research may produce no results at all.
